Mishap Involving U.S. Air Force F-16 Causes $30 Million in Damages Due to Maintenance Errors

Air Force Accident Investigation Board report

A runaway bomb lift rammed into a parked U.S. Air Force F-16 in the Middle East last November, resulting in $30 million in damages.

An investigation revealed that a maintainer lost situational awareness and mistakenly hit the accelerator instead of the brake. Contributing factors included insufficient documentation for bomb lift vehicles, a lax adherence to Air Force procedures, and rushed operations. The F-16 suffered extensive damage, while the F-15E involved incurred minor scratches.

This incident highlights ongoing concerns about maintenance practices in the Air Force, prompting new safety training initiatives.
